There is a specific category of body frustration that belongs exclusively to people who are doing everything right. They exercise regularly. They eat well. Their overall health is good and their weight is at or near where they want it to be. And yet there are specific areas of their body where fat simply does not respond to any of it. The inner thighs. The flanks. The lower abdomen. The area beneath the chin. The fat in these areas decreases somewhat with weight loss but never goes away entirely, and no amount of targeted exercise changes the underlying deposit.
This is not a failure of discipline or consistency. It is an anatomical reality. Fat cells in certain areas of the body are genetically predisposed to persist regardless of overall body composition changes. Liposuction removes these cells permanently, which is why it produces results that diet and exercise alone cannot replicate. It does not improve on a result that lifestyle could achieve, given enough time. It achieves a result that lifestyle cannot achieve at all.
Dr. Vickram Tandon, board-certified plastic surgeon at Tandon Plastic Surgery in Boston, performs liposuction as a contouring procedure for patients who are close to their goal and looking to refine specific areas of the body that have not responded to their efforts.
What Liposuction Is and What It Is Not
Liposuction is a surgical procedure in which a thin cannula is inserted through small incisions in the skin and used to physically remove fat cells from beneath the skin surface. The cannula is connected to a vacuum device that extracts the dislodged fat. Modern liposuction techniques, including tumescent liposuction in which the treatment area is first infiltrated with a dilute anesthetic solution, have made the procedure safer, more precise, and associated with less bleeding and faster recovery than older techniques.
What liposuction is not is a weight loss procedure. Patients who have significant weight to lose should achieve a stable weight before considering liposuction. The procedure is designed for patients who are at or near their goal weight and have specific anatomical areas of fat that are disproportionate to their overall body composition.
Liposuction is also not a treatment for loose skin. In patients with significant skin laxity, removing fat without addressing the overlying skin can worsen the appearance of the treated area. Dr. Vickram Tandon assesses skin quality and elasticity during the consultation and advises patients honestly about whether liposuction alone will achieve the desired result or whether a skin-tightening procedure should be considered in combination.
Areas Commonly Treated with Liposuction
The most commonly treated areas include the abdomen and flanks, the inner and outer thighs, the upper arms, the back, including the bra line area, the chest in male patients with gynecomastia, and the submental area beneath the chin. Submental liposuction, which addresses the fat deposit beneath the chin that creates the appearance of a double chin, is one of the most rewarding procedures in terms of the dramatic impact a relatively small intervention can have on facial appearance and profile. Patients who have struggled with submental fullness despite a lean overall body composition are often excellent candidates for this targeted procedure.
The Consultation with Dr. Vickram Tandon
The first step toward liposuction at Tandon Plastic Surgery is a consultation with Dr. Tandon, during which he assesses the patient’s overall health, evaluates the specific areas of concern, and determines whether liposuction is the most appropriate procedure for the desired outcome.
This assessment includes evaluation of the fat deposit itself, including its volume and distribution; the quality and elasticity of the overlying skin; the patient’s overall body composition and proximity to their goal weight; and any relevant medical history that may affect surgical candidacy or anesthesia management.
Dr. Tandon discusses realistic outcomes, explains which technique and approach he recommends for the specific anatomy, reviews the recovery process in detail, and answers all patient questions before any surgical planning is finalized.
Recovery and Results
Liposuction at Tandon Plastic Surgery is performed on an outpatient basis under general anesthesia or local anesthesia with sedation, depending on the extent of the procedure. Patients are discharged the same day and wear a compression garment over the treated areas for several weeks to support healing and minimize swelling.
In the first two weeks, swelling and bruising in the treated areas are significant, and the final result is not visible. Most patients return to desk work within one week and resume exercise at four to six weeks, depending on the areas treated.
Swelling can persist for three to six months after liposuction before the final result is fully apparent. Patients who evaluate their outcome before the swelling has fully resolved often underestimate their final result. The fat cells that have been removed are permanently gone, and as long as the patient maintains a stable weight, the improvement in contour is lasting.
